Overview
Dulaglutide (LY2189265) is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist developed by Eli Lilly. Structurally, it consists of two identical GLP-1 analogue chains, each covalently linked through a small peptide linker to a modified human IgG4 Fc fragment. This fusion design increases molecular size, limits renal clearance and slows degradation, giving the molecule a markedly longer half-life than native GLP-1.
Dulaglutide is an approved prescription medicine for type 2 diabetes in the United States, the European Union and many other regions, first authorised in 2014. It was evaluated in the AWARD clinical programme and in REWIND, a large cardiovascular outcome trial notable for enrolling a population in which most participants did not have established cardiovascular disease. For researchers, dulaglutide is a well-characterised example of Fc-fusion half-life extension and a benchmark in comparative GLP-1 receptor pharmacology, glycaemic research and cardiovascular outcome science. It also illustrates how protein engineering choices, such as Fc mutations that reduce immune effector function, are made in incretin drug design.
Mechanism studied
- Dulaglutide
- GLP-1 receptor
- Insulin secretion
- Glucose endpoints
Each GLP-1 analogue chain in dulaglutide carries amino acid substitutions that confer resistance to dipeptidyl peptidase-4 (DPP-4) cleavage and reduce immunogenicity, while the IgG4 Fc portion is modified to limit Fc gamma receptor binding and half-antibody formation. The molecule activates the GLP-1 receptor, a class B G protein-coupled receptor, stimulating Gs-cAMP signalling. In pancreatic beta cells this enhances glucose-dependent insulin secretion; in alpha cells glucagon release is reduced in a glucose-dependent manner. GLP-1 receptor activation also slows gastric emptying and engages brainstem and hypothalamic circuits involved in satiety. Its large size is thought to limit direct access to some brain regions.
Research highlights
- 01Developed by Eli Lilly and first approved by the FDA and EMA in 2014 for type 2 diabetes.
- 02Exemplifies Fc-fusion technology for extending the half-life of a short-lived peptide hormone.
- 03The REWIND cardiovascular outcome trial enrolled a broad population, including many participants without prior cardiovascular events.
- 04The AWARD programme compared dulaglutide with metformin, sitagliptin, exenatide, liraglutide and insulin glargine in head-to-head designs.
- 05Open questions include how large Fc-fused agonists access central satiety circuits compared with smaller acylated peptides.

FAQ
The Fc fragment from human IgG4 enlarges the molecule beyond the renal filtration threshold and allows recycling through the neonatal Fc receptor. Together these features slow clearance substantially. Mutations within the Fc region reduce its ability to engage immune effector pathways.
Dulaglutide is classified as a GLP-1 receptor agonist and, structurally, as an Fc fusion protein. It is an approved prescription medicine for type 2 diabetes. It is not an insulin and does not act directly on the insulin receptor.
REWIND was a long-term, placebo-controlled cardiovascular outcome trial of dulaglutide in adults with type 2 diabetes. It is often cited because of its unusually long follow-up and a study population in which the majority had cardiovascular risk factors rather than established disease.
