Active fragment of parathyroid hormone with anabolic bone activity

Reference compound — not available from Purity Peptides. Shown for research context.

Overview

Teriparatide is recombinant human parathyroid hormone fragment 1-34, the N-terminal sequence of the 84-amino-acid hormone that contains the activity required for receptor activation. It was developed by Eli Lilly and approved by the FDA in 2002, becoming the first bone-building, or osteoanabolic, agent for osteoporosis. It is an approved prescription medicine for osteoporosis in people at high fracture risk, and biosimilar versions are now available in several markets.

Teriparatide is central to one of bone biology's most important paradoxes. Continuous elevation of parathyroid hormone, as in hyperparathyroidism, promotes bone resorption, yet intermittent exposure stimulates bone formation more than resorption, producing net bone gain. The Fracture Prevention Trial, published in 2001, established the clinical relevance of this concept. Early rodent carcinogenicity studies found osteosarcoma in rats given long-term high exposure, leading to a boxed warning in the US that was later removed. Researchers use teriparatide to study bone anabolism, PTH1 receptor signalling, osteoblast lineage biology and fracture outcomes.

Mechanism studied

  1. Teriparatide
  2. PTH1 receptor
  3. Osteoblast activation
  4. Bone formation

Teriparatide activates the parathyroid hormone 1 receptor (PTH1R), a class B G protein-coupled receptor expressed on osteoblasts, osteocytes and renal tubular cells. PTH1R couples to Gs-cAMP-protein kinase A and to Gq-phospholipase C signalling. In bone, intermittent activation increases osteoblast number and activity by stimulating differentiation of precursors, reactivating quiescent lining cells and reducing osteoblast apoptosis. It also suppresses osteocyte expression of sclerostin, an inhibitor of Wnt signalling, and influences RANKL and osteoprotegerin expression. The early dominance of formation over resorption markers is often called the anabolic window. In the kidney, PTH1R activation enhances calcium reabsorption and phosphate excretion.

Research highlights

  • 01Approved by the FDA in 2002 as the first osteoanabolic agent for osteoporosis.
  • 02The intermittent-versus-continuous PTH paradox remains a foundational concept in skeletal biology.
  • 03Rat osteosarcoma findings shaped regulatory history; the US boxed warning was removed in 2020.
  • 04The Fracture Prevention Trial and later comparative studies made teriparatide a reference arm in bone research.
  • 05Open questions include how signalling duration at PTH1R determines anabolic versus catabolic outcomes.

FAQ

  • The designation refers to amino acids 1 through 34 of the 84-residue parathyroid hormone. This N-terminal fragment contains the residues needed to bind and activate the PTH1 receptor, so it reproduces the principal skeletal and renal signalling of the full hormone.

  • Brief receptor activation favours osteoblast recruitment, survival and sclerostin suppression, while sustained activation favours RANKL-driven osteoclast formation. The timing of PTH1R signalling therefore determines whether formation or resorption dominates, a principle extensively studied in rodent and cell models.

  • Teriparatide is a parathyroid hormone analogue and an osteoanabolic agent. It is an approved prescription medicine for osteoporosis in people at high fracture risk. It contrasts with antiresorptive agents, such as bisphosphonates, which act mainly by reducing osteoclast activity.
