Calcium-regulating thyroid hormone with now-restricted clinical use

Reference compound — not available from Purity Peptides. Shown for research context.

Overview

Calcitonin is a 32-amino-acid peptide hormone secreted by the parafollicular C-cells of the thyroid gland in mammals and by the ultimobranchial bodies in fish and birds. It was discovered in 1962 by Harold Copp, who identified a factor that lowered blood calcium, and is encoded by the CALCA gene, which through alternative splicing also produces calcitonin gene-related peptide (CGRP).

Salmon calcitonin is considerably more potent in mammals than the human hormone, owing to higher receptor affinity and slower dissociation, and it became the basis for most pharmaceutical products. Calcitonin is an approved prescription medicine in the US and EU, but its use was restricted after a 2012 European review associated long-term use with a small increase in malignancy risk; authorised uses in Europe now focus on Paget's disease of bone, hypercalcaemia of malignancy and acute bone loss from sudden immobilisation. In research, calcitonin is a classic tool for osteoclast biology, calcium homeostasis and class B receptor pharmacology, and serum calcitonin is an important tumour marker in medullary thyroid carcinoma research.

Mechanism studied

  1. Calcitonin
  2. Osteoclast receptor
  3. Bone resorption ↓
  4. Serum calcium ↓

Calcitonin binds the calcitonin receptor, a class B G protein-coupled receptor highly expressed on osteoclasts and also present in kidney and brain. Receptor activation couples to Gs, raising cAMP, and to Gq, activating phospholipase C. In osteoclasts this causes rapid cytoplasmic retraction, loss of the ruffled border and reduced secretion of acid and proteolytic enzymes, sharply decreasing bone resorption and calcium release. In the kidney, calcitonin increases calcium and phosphate excretion. When the calcitonin receptor associates with RAMP proteins, it forms amylin receptors, linking calcitonin and amylin pharmacology. Analgesic effects reported in bone pain models may involve central serotonergic and opioid pathways.

Research highlights

  • 01Discovered by Harold Copp in 1962; the term thyrocalcitonin reflected identification of its thyroid source.
  • 02Salmon calcitonin's greater potency in mammals made it the reference molecule for pharmaceutical development.
  • 03A 2012 European Medicines Agency review restricted long-term use after analysis of malignancy data.
  • 04Alternative splicing of CALCA to produce calcitonin or CGRP is a textbook example of tissue-specific RNA processing.
  • 05Open questions include the physiological importance of calcitonin in adult humans, since thyroidectomy does not markedly alter calcium balance.

FAQ

  • Salmon calcitonin binds the mammalian calcitonin receptor with higher affinity and dissociates more slowly than human calcitonin, producing a more potent and sustained response. Its stability and potency made it the standard molecule for pharmaceutical products and much experimental research.

  • Both are encoded by the CALCA gene. In thyroid C-cells, RNA processing favours calcitonin, while in neurons alternative splicing produces calcitonin gene-related peptide. The two peptides act on related receptors, with CGRP signalling depending on RAMP1 partnering with the calcitonin receptor-like receptor.

  • In 2012 the European Medicines Agency reviewed data from controlled studies and concluded that long-term use was associated with a slightly increased risk of malignancy. Authorised use was limited to short-term indications, and certain long-term osteoporosis uses were withdrawn.
